Health Intervention for Families
Food is a critical part of a patient’s healthcare! A healthy diet is impactful in the treatment and management of chronic illness, such as diabetes, high blood pressure, and cardiovascular. The Produce Prescription (PRx) Program connects eligible patients to providers who write prescriptions for free fruits and vegetables. Patients are screened for food insecurity and providers issue monthly prescriptions to meet the entire family’s recommended daily servings of fruits and vegetables.

Columbus
The Nurse Family Partnership in Franklin County is focused on providing care for pregnant patients and their newborns. The programs’ goals aim to improve health outcomes for up to 50 patients and their family members. Patients receive deliveries of produce or monthly prescriptions to shop for fruits and vegetables at farmers’ markets, neighborhood grocery stores, and other community-based food access points.
