Health Intervention
for Families

Your healthcare provider asks you to join a PRx program.

Attend regular check-ins with your program manager.

Each month you will check in with your healthcare provider and receive prescriptions for produce.

Redeem your prescriptions for fruits and vegetables at participating markets and grocery stores.

Food is a critical part of a patient’s healthcare! A healthy diet is impactful in the treatment and management of chronic illness, such as diabetes, high blood pressure, and cardiovascular. 





The PRx Program connects eligible patients to providers who write prescriptions for free fruits and vegetables.

Patients are screened for food insecurity and providers issue monthly 'prescriptions' to meet the entire family’s recommended daily servings of fruits and vegetables.

Produce Prescription (PRx) programs provide a medical intervention or prevention service for eligible patients that are nutrition insecure and have diet-related health risks.
