A digital self-attestation form from Ohio Department of Jobs and Family Services must be completed in person at coupon distribution events in order to receive fruit and vegetable coupons. The digital self-attestation form will ask individuals a series of questions to confirm they meet the eligibility criteria described above. If an individual is not eligible, the digital self-attestation form will close. If an individual is eligible, the individual will be able to continue the self-attestation form and provide additional required information such as name, social security number, and address, children’s names and ages.
