Complete a signup form in person at the Phoenix Earth Food Cooperative Friday, April 24th 1pm-3:00pm. No need to bring documentation, you will complete a form in person. Check your eligibility below!
✅ Who is Eligible?
To qualify, participants must meet all of the following criteria:
✅ Be a U.S. citizen or meet a citizenship exception
✅ Have a minor or pregnant woman in the household
✅ Meet the income limits listed below:
