Where: Addyston Healthy Harvest Mobile Market Stop: 235 Main St., Addyston

Complete a signup form in person at the Addyston Healthy Harvest Mobile Market Stop Tuesday, April 28th 1:30pm-2:30pm. No need to bring documentation, you will complete a form in person. Check your eligibility below!

📢PLEASE NOTE: This is a sign up event only, there will be no booklets at the event. Produce Perks Midwest will mail customers booklets after they've signed up with the Hamilton County Public Health Navigators.

✅ Who is Eligible?

To qualify, participants must meet all of the following criteria:

✅ Be a U.S. citizen or meet a citizenship exception
✅ Have a minor or pregnant woman in the household
✅ Meet the income limits listed below:
